什麽是天灸?What is Tian Jiu Moxibustion?

天灸是中醫傳統「灸法」中的一種。是指透過選用某些對皮膚有溫熱作用的藥 物敷貼於人體的穴位或患處,利用藥物的刺激作用,引起穴位局部皮膚充血, 甚至起泡,達到「溫經散寒,疏通經絡」的作用,起到「未病先防」和治療 「虛寒性疾病」的作用。

Tian Jiu Moxibustion is a type of traditional Chinese medicine therapy. It involves applying certain medicinal substances with warming effects to specific points or areas on the body. The stimulation from the medicine causes local skin congestion, and even blistering, at the points of application. This achieves the effect of “warming the meridians, dispelling cold, and unblocking the meridians,” serving as a preventative measure and treating “Yang deficiency diseases”(Diseases that are commonly affect by cold, such as Nose allergy, Asthma, Body pain, intestinal or menstrual situations etc.). 

 

爲何要選用特定日子?爲何分爲「夏天天灸」及「冬天天灸」? Why choose specific days? Why are there “Summer Moxibustion” and “Winter Moxibustion”? 

古人在長期觀測中總結出「冷在三九,熱在三伏」的規律。「三伏天」就是夏天 中陽氣最盛的日子,而「三九天」就是冬天最陰盛的日子。中醫認為這兩個時 期進行中藥敷貼療法效果最好。趁一年中陽氣最盛及最弱的時機,提升人體陽 氣,以「溫經通絡」的方法調整身體,增強免疫力,從而減低冬天疾病發作的 機會。

Ancient people summarized the rule of “cold in Sanjiu, heat in Sanfu” through long-term observation. “Sanfu” refers to the hottest days in summer, while “Sanjiu” refers to the coldest days in winter. Traditional Chinese medicine believes that the effects of medicinal patch therapy are best during these two periods. By taking advantage of the times when Yang energy is strongest and weakest throughout the year, it enhances the body’s Yang energy, adjusts the body through the method of “warming the meridians and unblocking the collaterals,” enhances immunity, and thereby reduces the chance of disease that onset in winter

 

哪些人適合接受天灸療法?Who is suitable for Moxibustion therapy?

中醫認為天灸通過溫熱藥物來刺激穴位,能增加體內陽氣,讓身體達到陰陽平 衡,故對「虛寒性疾病」或「冬季易發病」,都有其好處及作用。 例如: 鼻敏感、哮喘、慢性咳嗽、體虛易感冒、腰痛、經痛、慢性腹瀉、經常怕冷、 容易手腳冰冷、腹部冰涼感、頭痛、胃痛、肩背關節痛等。

Traditional Chinese medicine believes that Moxibustion stimulates acupoints through warm medicines, increases the body’s Yang energy, and achieves a balance of Yin and Yang in the body. Therefore, it has benefits and effects on “Yang-deficiency diseases” or “diseases that are easy to occur in winter.” For example: Nasal allergies, asthma, chronic cough, physical weakness and easy to catch a cold, back pain, menstrual pain, chronic diarrhea, often afraid of cold, easy to have cold hands and feet, abdominal cold feeling, headache, stomach pain, shoulder and back joint pain, etc. 

 

哪些人不適合接受天灸療法?Who is not suitable for Moxibustion therapy? 

患有以下急性疾病或特殊身體情況都不適宜進行天灸療法, 包括: 哮喘急性發作期、發燒、咽喉發炎、兩歲以下幼兒、「實熱、虛熱體質」者、孕 婦、肺結核、嚴重高血壓、糖尿病控制不當、嚴重心肺功能不全、瘢痕體質、 對中西外用藥過敏者。

Those with the following acute diseases or special physical conditions are not suitable for Moxibustion therapy, including: Acute asthma, fever, throat inflammation, children under two years old, “Heat, Yin-deficiency physique” people, pregnant women, tuberculosis, severe hypertension, poorly controlled diabetes, severe cardiopulmonary dysfunction, scar physique, and those allergic to Chinese and Western topical medicines.

敷貼後會有什麽反應?有副作用嗎?What reactions will there be after application? Are there any side effects?

敷貼處一般會出現皮膚發紅、輕微的發暖灼熱痕癢感覺,少數求診人可能感到 刺痛或起小水泡。部份人會於敷貼處出現色素沉著,一般數星期至數月後會自 行消退。 若果皮膚出現水泡,應好好保護表面,避免抓破引起感染;若有紅腫不適,也 可塗上護膚膏紓緩皮膚刺激。 只要遵從醫師的叮囑,並於建議時間內除藥,出現不良反應的機會是十分少。

The application site will generally appear red, with a slight warm, burning, itching sensation, and a few patients may feel stinging or small blisters. Some people will have pigmentation at the application site, which will usually fade on its own after a few weeks to a few months. If blisters appear on the skin, the surface should be well protected to avoid scratching and causing infection; if there is redness and discomfort, you can also apply a skin ointment to soothe skin irritation. As long as you follow the doctor’s instructions and remove the medicine within the recommended time, the chance of adverse reactions is very small.

 

藥貼可以敷貼多久?可以全日敷貼嗎?How long can the medicine be applied? Can it be applied all day?

一般成年人只需敷藥約 1 小時,小孩則按情況敷藥 15–30 分鐘。由於敷藥辛 溫,敷貼部位有機會有暖和感或灼痛感,皮膚亦有可能出現紅腫、刺痛感。雖 然以上皮膚反應均屬正常現象,但若真的難以忍受,緊記立即撕下敷貼。而過 久的貼敷,可能會令部分求診人士更容易出現水泡、皮膚破損等反應。

Adults generally only need to apply the medicine for about 1 hour, and children apply the medicine for 15-30 minutes depending on the situation. Because the medicine is warm-nature, the application site may have a warm feeling or burning sensation, and the skin may also appear red and have a stinging sensation. Although the above skin reactions are normal, if they are really unbearable, remember to tear off the application immediately. And too long application may make some patients more likely to have blisters, skin damage and other reactions.

貼藥後我要注意什麽?如何提高療效及減低出現不良反應的機會?What should I pay attention to after applying the medicine? How to improve the efficacy and reduce the chance of adverse reactions? 

  • 飲食配合:為免影響天灸的療效及好處,治療期間亦應盡量戒口,少吃易 化膿的「發物」(如牛肉、公雞、花生、燒鵝、燒鴨、芋頭等)、濕熱的食 物(如芒果、榴蓮等)、易致敏的食物(如魚、蝦、蟹等)、油膩煎炸食 物、辛辣食物及生冷食物等;
  • 皮膚護理:接受治療當天,敷藥部位不應以冷水冲洗或用肥皂等刺激性物 品擦洗。除藥後,應以濕巾簡單清潔敷藥部位,避免藥物殘存於皮膚過久 而灼傷皮膚。若有紅腫不適,也可塗上護膚膏以紓緩皮膚不適感。
  • 個別出現皮膚過敏嚴重者,如起較大水泡、水泡量多,或水泡破潰 者,應尋求醫護人員協助處理。
  • Dietary cooperation: To avoid affecting the efficacy and benefits of Moxibustion, you should also abstain from some king of foods as much as possible during the treatment period, eat less “pus-forming foods” (such as beef, rooster, peanuts, roast goose, roast duck, taro, etc.), damp-heat foods (such as mango, durian, etc.), allergenic foods (such as fish, shrimp, crab, etc.), greasy fried foods, spicy foods, and raw and cold foods;
  • Skin care: On the day of treatment, the application site should not be washed with cold water or scrubbed with irritating items such as soap. After removing the medicine, you should simply clean the application site with a wet wipe to avoid leaving the medicine on the skin for too long and burning the skin. If there is redness and discomfort, you can also apply a skin ointment to soothe skin discomfort.
  •  Individuals with severe skin allergies, such as larger blisters, multiple blisters, or broken blisters, should seek the assistance of medical staff for treatment.
